To indicate how you would like to receive your doctoral degree, you must first log in to the portal (by clicking on the "Login" tab on the left, logging in with your eduID) and then fill in the questionnaire (the questionnaire is accessible by clicking on the "Apply for the event" button at the bottom of the page).
